What to verify, and why each line matters
Annual maximum and what's left
The single number that decides whether a treatment plan gets paid this year or slips into the next one.
Deductible, and whether it applies to preventive
Many plans waive it for cleanings and exams. If yours doesn't, the patient owes something at a recall visit.
Coverage percentage by category
Plans differ most on basic and major work. Get each category, not one blended number.
Frequencies and history
A cleaning or bitewing done too soon is denied. Ask when each was last paid, not only how often it's allowed.
Waiting periods and the missing tooth clause
New plans often wait months before paying for major work, and a missing tooth clause can exclude replacing a tooth lost before coverage began.
Downgrades
Some plans pay a tooth-colored filling on a back tooth at the cheaper amalgam rate. The patient pays the difference, so they should know in advance.
Who you spoke to
Write down the rep's name and the call reference number. It's what you point to if a claim is denied for a benefit you were quoted.
Let an AI make the insurance verification calls
We're building an AI that calls the insurer, waits on hold, and fills in this breakdown, so your front desk isn't stuck on the phone. It isn't live yet. Join the invite list and we'll invite dental offices first.

FAQ
- What is dental insurance verification?
- It's checking a patient's coverage before the appointment: that the plan is active, what it pays for each kind of treatment, how much of the yearly maximum is left, and any limits like frequencies or waiting periods. Offices do it so treatment estimates are accurate and claims aren't denied.
- What is a dental insurance breakdown?
- Another name for the same record: the full list of a patient's benefits, written down in one place. This form is a breakdown template.
- When should a dental office verify insurance?
- A few business days before every new patient visit, and again before major treatment or whenever a patient says their plan changed. Many offices also re-check recurring patients at the start of each benefit year.
- Can I verify dental insurance online instead of calling?
- Often, yes. Most large carriers have a provider portal that shows eligibility and basic coverage. Details like history, downgrades, the missing tooth clause or ortho used to date are often missing there, so offices still call for those, which is where the long holds come in.
- What should I write down from the call?
- The rep's name, the date and the call reference number, alongside the benefits. If a claim is later denied for something you were quoted, that reference is your evidence.
